New Passport
Obtain a new passport if you have never held a passport, your passport was not issued in the last fifteen years, is no longer usable due to damage or alteration, or your passport was issued when you were younger than the age of sixteen
Passport Renewal
Renew your passport without the need to visit an acceptance agent if you received your current passport within the last fifteen years, you were sixteen years of age when it was issued, you still have your passport, and it is undamaged and in good condition. If your name has changed, you will need to provide legal documentation of your name change.
Lost/Stolen Passport
If your passport was lost or stolen, you will need to complete the application process for a new passport. To protect your identity, also file form DS-64 with the State Department as part of the application process to alert them to the loss of your passport.
Child Passport
For children fifteen years of age and younger, parents' proof of identity is required along with the child's proof of citizenship.
